Patient was apparently asymptomatic one and half year back then she complain of pedal edema one and half year back which is pitting type ,not aggrivated or relieved on medication.
Complain of SOB grade 3 on walking some distance and relieved on medication at hospital.
Also complain of facial puffiness and relieved on medication now complain of SOB since 7 days NYHA grade 3 not aggravated on talking no relieving factors ,complain of fatigability since 7 days but she can do all her household work no aggravation ....Complain of generalized weakness since 7 days distal muscle weakness greater than proximal .complain of tingling and numbness since one month (pcds and needle )distal greater than proximal...complain of chest pain since 5 days non diffuse, intermittent ,non radiating aggravated factor- increase of work but relieved on rest..no complain of palpitation syncopal attacks ,no complain of pain abdomen ,abdominal distension ,burning micturition ...no complain of cough, cold orthopnea ,PND.
No history of previous blood transfusion.
No history of DM, HTN, TB, ASTHMA, EPILEPSY, THYROID.
PAST HISTORY :
SIMILAR C/O 1 AND HALF YEAR BACK, RELIEVED ON MEDICATIONS.
MENSTRUAL HISTORY:
3/28, regular., no clots...
Personal history :mixed, regular bowel and bladder normal, no addiction..
Pallor PRESENT, NO icterus, no pedal edema.
On Examination :

USG abdomen and pelvis:
2) BLOOD TRANSFUSION DONE ON 28/2/21 ONE UNIT OF PRBC.
Day 2
1) TAB. PANTOP PO/OD
2) BUSCOPAN 1 AMPOULE /IV/SOS
3) SYRUP CREMAFFIN 15 ML PO/HS.
PLAN FOR BLOOD TRANSFUSION.
Day 3
1) TAB. PANTOP PO/OD
2) BUSCOPAN 1 AMPOULE /IV/SOS
3) SYRUP CREMAFFIN 15 ML PO/HS.
PLAN FOR BLOOD TRANSFUSION.
4) INJ. VIT B12 1000MICROGRAM I'M/DAILY FOR 1 WEEK.
Patient wants to leave to hyderabad, even on request on not willing to stay ahead fr treatment.
